I'm trying to get my funding approved from pct to get bariatric surgery at st richards. had it denied once already :cry:and want to make sure all ok this time. just received these questionnaires by post on self-esteem. Anyone knows what they are about??

Hi RHJ, and welcome to Mini's.

I havent had surgery at St Richards myself, but I did read a post the other day saying they had closed the waiting list at the moment.

Do you know which PCT (primary care trust) you come under? If so, someone from your area may be able to answer your questions.
 
Hi, it sounds like a pre-psyche evalution questionnaire. It will just be to get an idea of your psychological readiness for dealing with surgery. Just answer them honestly and you'll be fine.
 
had my op at st richards on the 22nd. list there is still closed at present to clear the backlog. forms do sound like the physch ones but bit odd if they are saying no to funding.xx
 
I had some forms to complete pre my funding at St Richards being approved back at the beginning of this year (January time). they werent psych forms. i can't remember what they were about in all honesty but they were I think part of checking your suitability for the operation and funding. Can I help at all?
 
Tks everyone for answers, I'm totally lost in all this paperwork :(

I filled in the papers now and, one I think it are psych forms, other is a 34 questionnaire on how I feel and dietary habits and, the last one I need to register my diet habits for 5 days. Does it rings abell to anyone??

I am not worried about waiting list being closed atm 'cos I'm already there. seen mr. sommers twice already. My case is bit weird cos I had a lapband 4 yrs ago that went wrong so, I had part of funding previously approved to remove my lapband and, mr sommers is now reapplying so I can have bypass alltogether in one surgery only as my bmi is +40. That is why it is so important to me to get everything right. I struggle every day with health problems not only from obesity but also cos of my wrongful lapband.

Mr sommers already placed me on waiting list for surgery, even though still waiting for funding approval and that was when I now got these questionnaires.

Any comments and thoughts more than welcome

Hi the one with the eating habits for 5 days does sound a little bit like the psych form (you have to put down what you ate, where you were when you ate it, what you were thinking, how it tasted etc).

One might be the dieticians form (have you seen the dietician?)

And the other one, I am bemused about, it was a long time ago when I filled out my funding form but the one you mention might well be the initial funding form . Does it ask you questions about what you have tried in the past, how much weight you have lost in the past ? If so that is the initial funding form but if not, then sorry, I can't help either.

Wish you luck with that journey, I'm nearly there, my pre op is 13th November and op 27th ...

Yes, it does do that sort of questions. What is that 'initial funding form'? shall I take it as a good sign I haven't immediately received a no letter like before? Really don't want to get hopes high again, cos the fall is big :(

In all honesty, I can't give you that answer, I'm so so sorry. I think they weigh (excuse the pun) each case on its' own merits. I would just complete the form totally honestly. They are there to help you. I was embarrassed to put everything on it and some of the questions I felt a little intrusive. But just be totally open and honest.
